If you are new to Pop Star Performers please could you fill out this form, giving us all the information about your child. 

Please include any info about your child’s previous experience, plus any details we need to know to help them settle and feel safe. You can never give us too much info!! 

If you are a current or past customer, you don’t need to do anything. 

We will send out an email with all the information you need 1 week before the event.
